Stress Journal

Format for journal (per stressor):

Day & Date (Time of day would be helpful too!!)
Potential Stressor (ex. traffic; meeting; wedding planning; Christmas shopping)
Threat (Describe what is threatening about this stressor)
Intensity Level (lowest = 1 & highest = 10)
Physical Response (what physical sensations were happening at the time)
Emotional Response (describe the emotions associated with the stressor)
Perceived Ability to Cope (could not cope at all = 1 through could completely cope well with the situation = 10)
Coping (Describe what you did to cope)
Coping Effectiveness (ineffective = 1 through very effective = 10)
How would you cope differently next time (If effectiveness was less than a 10)
